KUCHING: Sarawak General Hospital (SGH) is appealing to the public to help locate the next of kin of a patient, Poh Tze Choon, who is due to be discharged from the hospital.
According to a statement issued on Tuesday (Jul 21), the patient has been receiving treatment at Medical Ward 5 since June 12 and is currently awaiting discharge.
The hospital said various efforts have already been made to trace the patient’s family members, but these have so far been unsuccessful.
Based on the patient’s admission records, the last known address is No. 22, Lorong 4, Jalan Star Garden, Jalan Green, 93050 Kuching.
The hospital also noted that the contact number obtained during the admission process is no longer reachable.
SGH is therefore urging Poh’s relatives or family members to come forward and contact its Medical Social Work Officer, Rita Fazlin Awang @ Ibrahim, at 082-276666 ext. 4374 during office hours.
Alternatively, family members may contact Medical Ward 5 at 082-276666 ext. 5934 as soon as possible.
