The public are adviced to bring along their own, their spouse’s and also their children’s identity cards (I.C) or birth certificates, latest pay slips, bank account information and beneficiaries’ identity information The crew will assist in filling in the forms for applications or information updates.
Applicants from single-parent families are advised to provide related information such as divorce certificates and death certificates to avoid their application being rejected by LHDN.
